Getting a Health Certificate in Davao City

Every person who is employed in any business establishment in Davao City is required to get a health certificate, also known as health card, from the City Health Office (CHO). From January to March this year, the CHO of Davao City has opened its satellite operations in the 2nd floor of Felcris Centrale Mall, fronting Land Transportation Office.

CHO caters to a huge number of applications and renewals every day since it started last January. So expect a really long line if you come in later than 12 noon. Make sure to be in Felcris Centrale before the mall opens at 10am. Some people queue as early as 6-7am just so they could finish early or within the morning.

Also, it's best to prepare the requirements ahead for a smooth application process. The requirements for a Health Certificate are as follows:

1. Application Form - The form is usually given upon queuing so always bring a pen so as to accomplish the said form.

2. Community Tax Certificate (CTC) - Also known as the Cedula, the CTC may be obtained from the Barangay, Municipal or City Hall where it is being issued. Currently, there is also a station conveniently located in the upper ground parking of the Felcris Centrale Mall, just in front of the elevator lobby, which issues CTC.

3. Official Receipt (OR) for Application/Renewal - The OR, similar to the CTC, is also being issued at the same station in the upper ground parking of the Felcris Centrale Mall. Application Fee for new health certificates/card is at P50 and late applications, renewal or replacement of lost/damaged card is at P100.

4. Normal Offical Results of the following medical exams:
     a. Chest X-ray or Sputum Test (if pregnant) done in the last 6 months
     b. Fecalysis done in the last 3 months
     c. Urinalysis done in the last 3 months
     d. Additional tests done in the last 12 months:
          i. Drug & Neuro-psychiatric test - Conductors, drivers, guards, teachers, etc
          ii. Hepatitis B Test - Pink card applicants
          iii. Parental Consent & ID - Applicants below 18 yrs old

5. Food Handling Seminar Certificate - There is also a food handling seminar being held in the Felcris Centrale Mall. You may ask the guards as to the time slot and specific location in the ground floor as they usually change their venue around the mall.

Once you have the complete requirements, you may proceed lining up in the upper ground parking of the Felcris Centrale Mall where the guards will send you up in the 2nd floor by batches to avoid crowding the City Health Office area. After a long queue, a receiving officer will receive and review the application and asses the validity and completion of your requirements. It will then be passed on to the nurse who will asses the findings of your medical documents. Once cleared, the nurse will provide a priority number for the encoding. You will go to the waiting area in front of the encoding section where you will wait for your priority number to be assigned to one of the encoding stations. Once called, present your documents to the encoder. The encoder will take your photo and encode your data in the system. Once the certificate is printed, you be asked to sign the certificate. After which, your signed certificate will be given to a medical officer for review and signature. Then the certificate is finally released to the applicant.


Also, there is a cut-off time of 5PM for submission of applications. Good luck on your application!

We were advised that CHO is no longer in Felcris Centrale for their satellite operations. Nonetheless, the process and requirements remain the same.
